• 4-person scramble — captain choice format
• 9 am check-in and mulligan sales, 10 am shotgun start, 2 pm lunch and awards

Registration and Sponsorship

• Entry fee $500/team and $150/single player. Online registration now open, with secure credit card payment option. Golfers can also pay by Venmo (@TheMyositisAssociation), PayPal, or check.
• Fee includes: Golf, cart, range balls, lunch & team / individual prizes!
• Hole sponsorships available, with tee box signage for $500.
• Can’t attend, but want to support TMA? Register as a virtual golfer for $100.
